Assumption of Risk

I, the undersigned parent or legal guardian of the above-named participant, hereby acknowledge that participation in baseball activities — including but not limited to tryouts, practices, games, team events, travel, and conditioning sessions — involves inherent risks. These may include injuries from thrown or hit balls, bats, physical contact, slips, trips, falls, weather exposure, and travel-related incidents. I understand and accept that my child’s participation, whether in tryouts or throughout the baseball season if selected, is voluntary and at his/her own risk.
I acknowledge the inherent risks of participation.

Waiver and Release of Liability

In consideration of my child being allowed to participate in the tryouts and, if selected, the full baseball season, I hereby release, waive, discharge, and hold harmless Hit and Run LLC d.b.a. PA Gamblers, its coaches, volunteers, directors, officers, affiliates, sponsors, field owners, and representatives (collectively, the “Released Parties”) from any and all liability for injury, illness, death, or property damage arising out of or related to my child’s participation — whether caused by the negligence of the Released Parties or otherwise. I release and hold harmless the organization from liability.

Covenant Not to Sue

I further agree that I will not initiate, support, or encourage any lawsuit or legal claim against the Released Parties for any cause of action or claim arising from or connected with my child’s participation in the baseball program, including tryouts, games, practices, team events, or travel. This agreement constitutes a binding covenant not to sue and is intended to bar all legal claims of any nature now or in the future. I agree not to sue the organization.

Medical Consent

I certify that my child is in good health and physically able to participate in all baseball-related activities. In the event of a medical emergency, I authorize team staff or designated personnel to obtain medical care for my child. I agree to bear all medical costs and assume all responsibility for any resulting treatment. I authorize emergency medical treatment.

Photo and Video Release (Optional)

I give permission for my child’s photo or video image to be used by Hit and Run LLC d.b.a. PA Gamblers for promotional, social media, or team purposes without financial compensation. I grant permission for photo/video use (optional).

Acknowledgment and Signature

I have read, understood, and voluntarily signed this waiver and covenant not to sue. I acknowledge that by signing this document, I am giving up legal rights on behalf of myself and my child and agree to be bound by its terms throughout the baseball season, beginning with tryouts and continuing through the final team activity.
